Optional Chiropractic Care
Not all plans cover chiropractors -- practitioners who manipulate the spine and other structures within the body to relieve pain and tension resulting from posture, stress or strain. Some plans offer chiropractic care as an optional benefit that you or your employees may choose to add at an additional cost. If this is an important benefit to you or your employees, you may want to consider a plan that offers this type of care.

Optional Maternity Care
Maternity care coverage typically includes OBGYN visits and hospitalization and physician fees associated with the birth of a child. Optional maternity care covered benefits may vary by insurer.
Optional Prescription Drug (Rx)
A type of specified expense coverage that provides benefits for the purchase of drugs and medicines prescribed by a physician and not available over-the-counter. Often a plan will provide a prescription drug card that allows the insured to obtain medications by simply paying a copay at a participating pharmacy.
Optional Supplemental Accident
This coverage provides first dollar coverage for you and your family in the event of accidental injury.
Optional term life
A type of life insurance that provides a death benefit if the insured dies during a specific period.
Optional Vision Care
A type of specified expense coverage that provides benefits for expenses the insured incurs in obtaining eye examinations and corrective lenses.
Other Monthly Fees
Some insurance carriers may charge an additional monthly fee per person covered by the plan. American Medical Security (AMS): United Wisconsin Life Insurance Company/American Medical Security (UWLIC/AMS) is available for Taxpayers Network Inc. (TNI) members only. UWLIC/AMS is authorized to collect TNI's monthly dues of $7.00 per month, which will be billed with premium. Golden Rule Insurance Company: These plans are available only to members of FACT. Golden Rule Insurance Company Estimated Base Monthly Premiums include the mandatory $3 per month dues for membership in the Federation of American Consumers and Travelers (FACT), an independent consumer organization.

Out-of-Pocket Maximum - Individual
A dollar amount set by the plan which puts a cap on the amount of money the insured must pay out of his or her own pocket for covered expenses over the course of a calendar year.
outpatient surgery
Surgery that does not involve an overnight stay in a hospital. Common outpatient surgeries include tonsillectomies, colonoscopies, biopsies and cataract removal.
